Missed GLP-1 dose guide: rules differ by product

One missed-dose rule cannot cover every GLP-1 treatment. Identify the brand, formulation, and last actual dose before deciding what to do next.

What to know: Never double a dose. Ozempic uses a 5-day window, tirzepatide a 4-day window, and Wegovy injection uses the time until the next scheduled dose.

Compare the labeled rules

The table summarizes common product instructions, but it is not a substitute for the prescription and current label. An exactly 2-day Wegovy boundary is not clearly resolved by the label's greater-than and less-than wording; ask the clinician or pharmacist rather than inventing an inclusive rule.

The late-dose window and the minimum interval between actual doses are different requirements. If a schedule is being changed at the same time as a missed dose, have a pharmacist check both.

Selected missed-dose rules from current prescribing information
ProductMissed-dose instruction
Ozempic injectionTake within 5 days; after that, skip
Mounjaro or ZepboundTake within 4 days (96 hours); after that, skip
Wegovy injectionTake if next dose is more than 2 days away; skip if less than 2 days away
Wegovy tabletsSkip missed tablet; take next dose the following day

Two missed doses are not always the same as one

The June 2026 Wegovy prescribing information calls for restarting escalation at a lower dose after two or more consecutive missed injections. Arrange this with the prescriber rather than returning automatically to the former dose.

For any prolonged interruption, ask how to restart the exact product. A previous maintenance dose does not prove current tolerability, and the reason treatment stopped may affect whether restarting is appropriate. Do not use another person's old starter pen or unlabelled supplies.

Separate uncertainty from a confirmed missed dose

If you cannot tell whether an injection was delivered, contact the pharmacist or prescriber. A repeated injection could give too much medicine. Keep packaging, product name, dose, and the time of the attempt available.

Use a log or reminder to reduce duplicate treatment, especially if more than one person assists with injections. The same tracking approach can help with daily tablets, but a tablet's rules still come from its own label.
