Zepbound and obstructive sleep apnea: the approved role

Zepbound is labeled for moderate-to-severe obstructive sleep apnea in adults with obesity. This is a specific clinical indication, not a claim that all tirzepatide products treat all sleep problems.

Who the label describes

The current Zepbound label specifies adults with obesity and moderate-to-severe OSA, alongside a reduced-calorie diet and increased physical activity. The diagnosis and severity should be established through the appropriate sleep-care pathway.

Mounjaro's indication is different despite containing tirzepatide. A compounded product is also a separate regulatory category. The label's sleep-apnea claim cannot be transferred automatically to either.

Maintenance differs by indication

Zepbound starts at 2.5 mg weekly and escalates through the labeled steps. Its recommended OSA maintenance doses are 10 or 15 mg weekly, with the choice guided by response and tolerability. A starter dose is not the OSA maintenance dose.

The clinician should manage escalation and symptoms. Do not rush increases to improve a sleep-study number, and do not continue an intolerable dose solely because it is within the labeled range.

OSA still needs ongoing care

Ask the sleep clinician how symptoms, PAP use, weight change, and any repeat testing will be assessed. Treatment plans can differ between people using PAP and those unable to use it, and an individual assessment remains necessary.

Improved symptoms do not automatically prove that OSA has resolved. Keep existing therapy coordinated rather than stopping a device or changing pressure settings based on a weight change alone.

The usual tirzepatide precautions remain

Gastrointestinal symptoms, dehydration-related kidney injury, pancreatitis concerns, and other warnings remain relevant. Review MTC/MEN 2 contraindications, diabetes medicines, pregnancy plans, and oral contraception with the prescriber.

Coverage for sleep-apnea treatment may require specific documentation. A clinical indication and an insurer's benefit decision are different questions, so verify the exact product and diagnosis in the authorization process.
