Foundayo dosage chart: initiation, escalation, and interactions

Foundayo's milligram strengths are specific to orforglipron. They cannot be converted from Wegovy tablets, Rybelsus, or injection doses.

The labeled daily schedule

Start at 0.8 mg daily. The first two increases are to 2.5 mg and then 5.5 mg after at least 30 days at each preceding strength. Higher steps can be selected according to treatment response and tolerability; there is no requirement that every patient reach the highest strength.

A slow response does not justify taking two tablets, splitting a tablet, or moving up without a prescription. Starter doses introduce treatment and help address tolerability. Discuss persistent nausea or other symptoms before the next scheduled increase.

Taking a tablet and handling interruptions

Take the tablet once daily with or without food and swallow it whole. Do not crush or chew it, and do not take more than one tablet per day. Unlike oral Wegovy, this product does not require the same morning fasting routine.

For a missed Foundayo dose, the label says to take it as soon as possible without doubling the next dose or taking more than one tablet in a day. After 7 or more consecutive missed doses, restart escalation at a lower dose with the prescriber. Product-specific directions matter more than a generic GLP-1 catch-up rule.

Why the maximum may be lower

With a strong CYP3A4 inhibitor, the label limits Foundayo to 9 mg daily. It advises avoiding strong inhibitors that also inhibit OATP1B and avoiding strong CYP3A4 inducers. Concomitant simvastatin should not exceed 20 mg daily. These are examples of why a complete pharmacist review is necessary, not a list to manage on your own.

Bring prescription medicines, nonprescription products, and supplements to the review. Pregnancy, hepatic disease, insulin or sulfonylurea use, and contraception also affect treatment planning. Oral contraception needs a non-oral method or added barrier protection for 30 days after starting and each escalation.
