GLP-1 medicines, pregnancy, and oral contraception

Pregnancy planning and contraception should be discussed before starting a GLP-1 medicine. The instructions differ by product and cannot be replaced by one class-wide rule.

Make a plan with the prescriber

Weight-management medicines are not a treatment for weight loss during pregnancy. The relevant labels advise actions when pregnancy occurs; discuss these before treatment and contact the prescriber promptly if pregnancy is suspected or recognized.

Semaglutide labels include discontinuation at least 2 months before a planned pregnancy because of its long washout period. The exact indication and clinical circumstances matter, so the clinician should coordinate diabetes or other care during a change in treatment. Do not stop all related medicines without a replacement care plan.

Tirzepatide and oral hormonal contraception

Mounjaro and Zepbound delay gastric emptying and can reduce oral hormonal contraceptive effectiveness. Their labels advise switching to a non-oral method or adding a barrier method for 4 weeks after initiation and for 4 weeks after every escalation.

Moving the injection to another hour does not remove the precaution. Ask which contraceptive method and backup approach fit your circumstances, and discuss vomiting or diarrhea that may affect oral medicine use.

Foundayo uses different wording

Foundayo's label advises a non-oral contraceptive method or added barrier protection for 30 days after starting and for 30 days after each dose escalation. This is a separate product-specific instruction, not a reason to use tirzepatide's exact 4-week rule for every pill.

Foundayo also has medicine interactions requiring review. Bring the full medication and contraception list to the prescriber or pharmacist rather than focusing only on the weight-management prescription.

Breastfeeding and restarting

Breastfeeding guidance is formulation-specific. The current Wegovy label advises against breastfeeding during tablet treatment; other products need review of their own labeling and the individual's circumstances.

After pregnancy, breastfeeding, or a prolonged interruption, discuss whether restarting is appropriate and which dose should be used. The pre-interruption dose should not be resumed automatically.
