Foundayo review: orforglipron's approved role in 2026

Foundayo contains orforglipron, a nonpeptide oral GLP-1 receptor agonist. FDA approved it on April 1, 2026 for adult weight management in the labeled population.

What approval covers

FDA approved Foundayo with a reduced-calorie diet and increased physical activity for adults with obesity, or overweight with at least one weight-related condition. Approval for that population should not be extended to children or presented as a diabetes indication.

Foundayo is a different molecule from semaglutide. The absence of food restrictions is an administration difference, not proof that it is safer or more effective than oral Wegovy. Using it together with another GLP-1 receptor agonist is not recommended.

Daily use and titration

The label starts at 0.8 mg once daily. After at least 30 days, the dose rises to 2.5 mg daily, then after at least another 30 days to 5.5 mg. Further steps of 9, 14.5, and 17.2 mg may be used after at least 30 days at the current level according to response and tolerability.

Take it with or without food and swallow the tablet whole. Do not take more than one tablet per day. The maximum general dose is 17.2 mg daily, but interaction-related restrictions can lower the permitted maximum. A pharmacist should review the complete medicine list.

Interactions need their own review

Foundayo has clinically relevant interactions involving CYP3A4 and other transport pathways. The label includes restrictions involving strong inhibitors and inducers, and a dose limit for concomitant simvastatin. Do not infer safety from another GLP-1's interaction list.

Oral contraceptive users are advised to switch to a non-oral method or add a barrier method for 30 days after initiation and for 30 days after every escalation. This differs from tirzepatide's 4-week wording. Discuss contraception and pregnancy plans before treatment.

Safety and realistic comparison

Foundayo carries a thyroid C-cell tumor boxed warning based on GLP-1 receptor-dependent effects observed with pharmacologically active products in rodents. Orforglipron itself was not pharmacologically active in rats or mice and did not produce rodent tumors. Human relevance of the class finding remains undetermined; personal or family MTC and MEN 2 are contraindications.

Important risks include pancreatitis, severe gastrointestinal reactions, dehydration-related kidney injury, hypoglycemia with insulin or secretagogues, allergy, gallbladder disease, and aspiration concerns during anesthesia. The label does not recommend use in severe gastroparesis or severe hepatic impairment. Trials of other drugs do not establish a direct ranking for Foundayo.
